Release Form: By the submitting of and or signing of and in consideration of acceptance of this application, I am intending to be legally bound for myself, my heirs, executors and administrators, do hereby waive and release all rights and claims for damages which might be sustained by me in connection with my participation or entry in this camp or tournament, and which may arise out of my traveling to, participating in, or returning from the event. I give permission for my child to videotaped or photographed in the camp environment. All disputes to be settled in the state of Florida. In the event of illness or injury, International Centers of Excellence are authorized to obtain any medical care or treatment deemed necessary.
